Family Centers Assumes Management of Adult and Pediatric Health Center at 75 Holly Hill Lane in Greenwich

GREENWICH, CONN. (February 4, 2025) – As of February 4, Family Centers has assumed management of the adult and pediatric primary care clinics formerly operated by Greenwich Hospital/Yale New Haven Health Systems at 75 Holly Hill Lane in Greenwich.
The move is part an agreement between Family Centers and Greenwich Hospital intended to enhance primary care services for local adults and children and improve health care access for underserved populations. Both organizations will leverage their clinical expertise and experience to establish a competent care model that delivers an optimal patient care experience. With Family Centers in the lead, patients will now have access to a comprehensive network of behavioral health, dental and human services to complement their medical care.
“Ensuring that all residents have access to quality medical care is an essential component in preserving the overall well-being of any community,” said Family Centers’ Chief Health Officer Dennis Torres. “This agreement with Greenwich Hospital not only expands primary care services but also integrates a network of wrap-around supports to better serve our community’s most vulnerable populations. By combining outstanding medical care with behavioral health and human services, we can offer a coordinated, holistic approach to care that improves patient health outcomes.”
The new facility at 75 Holly Hill Lane features 15 exam rooms and will increase Family Centers’ capacity to meet patient needs by 20 percent. With mental health services embedded on-site, patients will benefit from a “warm hand-off” to mental health clinicians.
Family Centers’ existing health center at 111 Wilbur Peck Court will continue to provide dental care services while continuing some primary medical care hours. There are plans to expand dental services after the health center transition is complete.
