Dear Family Centers Patients:
We are excited to announce that as of February 2025, our medical services at 111 Wilbur Peck Court have moved to a larger health center at 75 Holly Hill Lane, Suite 102, in Greenwich. This new facility has 15 exam rooms and separate wings for pediatric and adult care, designed to offer a better experience for all our patients.
What You Can Expect at Our New Health Center:
• More Space: The new center has five times more exam rooms than our current location. This means we can offer quicker and more efficient care to both adults and children.
• Support from Yale New Haven Health/Greenwich Hospital: Our new health center is a collaboration with Yale New Haven Health and Greenwich Hospital. Their doctors, specialists, and providers will work closely with our team to ensure the highest quality care.
• Expanded Services: With more space and new equipment, we’ll be able to perform more in-house tests and screenings. We’ll also begin offering Family Centers’ Behavioral Health services at this location, providing a full range of care. Our care coordinators and referral specialists will help manage your care for the best health outcomes.
• 24/7 Access to Medical Records: You will still have access to your medical records through Epic’s MyChart, allowing you to view your health information at any time, day or night.
Comprehensive Care for All Ages:
• Medical & Mental Health Services: Our new health center will provide both medical and mental health services for children, adults, and seniors.
• Bilingual & Translation Services: We will continue to have bilingual staff and translation services to ensure clear communication for all patients.
• Pharmacy Partnership: We’ve partnered with Curant Health Pharmacy, which offers home delivery of medications and case workers to help manage your prescriptions.
Dental Care will continue to be offered at our 111 Wilbur Peck Court location in Greenwich, and we’re planning to expand these services even more very soon. We will also keep providing access to all of Family Centers' other wrap-around health care, social, and education services. The new health center will include additional meeting space where you can consult with our care managers to explore other services that may benefit you.
