Family Centers Health Care Earns National Quality Accolades
Family Centers Health Care (FCHC), a federally qualified health center based within the Wilbur Peck Court, a housing development of Greenwich Communities, was recognized for outstanding quality by the U.S. Department of Health and Human Services (HHS).
HSS recently announced the recipients of its 2020 Quality Improvement Awards, which celebrates the highest performing health centers nationwide and those that have made significant quality improvements from the previous year. FCHC’s five HSS Quality Improvement Awards were accompanied with a $77,201 grant award, which was distributed through the Health Resources and Services Administration (HRSA). More than $117M in quality improvement grants were allocated to 1,318 HRSA-funded health centers across the U.S. to further strengthen quality improvement activities and expand quality primary health care service delivery.
“We are extremely grateful for this recognition from HHS and HRSA for the high level of quality care we provide,” said Dennis Torres, Family Centers’ Chief Health Officer. “Providing excellent care to the more than 1,200 children and adults who rely on our services each year is a top concern. This funding ensures our team can implement additional quality improvement activities so that our community can continue to benefit from top-notch care as we continue to manage the COVID-19 pandemic.”
FCHC was recognized in the following categories:

Improving Quality of Care – Recognizes health centers that improved quality of care between 2018 and 2019.

Access Enhancer – Recognizes health centers that increased the total number of patients served and the number of patients receiving comprehensive services between 2018 and 2019.

Value Enhancer –Recognizes health centers that improved cost efficient care delivery as compared with the national average, while also increasing quality of care and improving patient access to comprehensive services between 2018 and 2019.

Advancing Health Information Technology (HIT) for Quality – Recognizes health centers that optimized HIT services for advancing telehealth, patient engagement, interoperability, and collection of social determinants of health to increase access to care and advance quality of care between 2018 and 2019.

Patient Centered Medical Home – Recognizes health centers with patient centered medical home (PCMH) recognition in one or more delivery sites.
A program of Family Centers, Family Centers Health Care is a federally qualified health center offering primary health care, dental and mental health services to children and adults living in Greenwich Public Housing communities, Section 8 housing and the surrounding neighborhoods. The clinic features medical examination rooms and a full dental suite. Mental Health services are provided at Family Centers’ Greenwich and Stamford offices.
