Ramin Ahmadi, MD
Medical Director
Dr. Ahmadi graduated from the Medical College of Wisconsin, studied Internal Medicine at Yale School of Medicine, and earned a Master of Public Health degree from Yale School of Public Health. He is a trusted medical and public health expert, and his insightful perspectives on healthcare have been featured in Healthline, Marketwatch, Cosmopolitan, and PBS Next Avenue. He has worked with numerous academic medical centers and community hospitals for more than two decades. As a clinician educator, he has been the recipient of many awards of excellence and has secured critical resources through highly competitive private funding receiving over $20 million dollars in federal grants to build accredited residency training programs in primary care.
Donna Phanumas, MD
Geriatrician
Dr. Donna Phanumas is a graduate of New York Medical College. She completed her internal medicine residency at North Shore University Hospital in Manhasset, NY and her geriatrics fellowship at Stony Brook Hospital in Stony Brook, NY. Her interests include teaching medical interns and residents at the Greenwich Hospital Internal Medicine Residency program and caring for the elderly population.

Ishrat Rangwala, DMD
Dentist
Dr. Rangwala received her Doctor of Dental Medicine (DMD) degree from University of Pennsylvania School of Dental Medicine. Prior to moving to United States, she received her Master in Dental Surgery (MDS), Prosthodontics from Gujarat University, India. She is a member of American Dental Association, Connecticut State Dental Association, National Network for Oral Health Access, and serves on the CT state’s CTDHP Dental Policy Advisory Committee. Dr. Rangwala was in private practice for over 5 years prior to joining Family Centers in 2016. Currently as Dental Director, she leads the dental program that serves to a diverse population. She excels in providing innovative, advanced, comprehensive care with strong emphasis on patient education and partnership. She is passionate about preventive dental care and integration of oral health with patient’s overall well-being. Each year, Dr. Rangwala completes over 50 hours of Continuing Education and has taken courses in prosthodontics, endodontics, oral surgery and infection control.
Charles Seelig, MD, MS, FACP
Adult Primary Care Provider; Director of Medical Education, Greenwich Hospital; Clinical Professor of Medicine at Yale School of Medicine; Clinical Adjunct Professor of Medicine at New York Medical College
Dr. Seelig is a board-certified physician with decades of experience in primary care, academic medicine, and medical education. A graduate of New York University and the University of Rochester School of Medicine, he completed his residency at Maine Medical Center before serving as a primary care physician in the U.S. Air Force for six years. Dr. Seelig joined the University of Nebraska faculty in 1984 and later served as Director of Ambulatory Care at the Coastal Area Health Education Center affiliated with the University of North Carolina at Chapel Hill. Since 1995, he has held a clinical leadership position at Greenwich Hospital. An active contributor to undergraduate and graduate medical education at the local, regional and national levels, Dr. Seelig received the Parker J. Palmer Courage to Teach Award from the ACGME in 2012 and the George F. Thornton Medical Education Award from the Connecticut Chapter of the ACP in 2015. He is enthusiastic about the collaboration between Greenwich Hospital and Family Centers to deliver exceptional patient care and support the clinical education of future physicians.
Jon Zimmerman, MD, MBA, FACP
Adult Primary Care Provider; Associate Program Director, Internal Medicine Residency Program at Greenwich Hospital/Yale New Haven Health System; Clinical Assistant Professor at Yale School of Medicine
Dr. Zimmerman graduated from the University of Michigan Medical School, completed his Internal Medicine Residency at Michigan Medicine, and received an MBA from the Tepper School of Business at Carnegie Mellon University. He has been a teacher and mentor to Internal Medicine faculty, residents and medical students for over 20 years. He has held several leadership positions, including Director of Educational Development, Internal Medicine Residency Program Director, and Chief of Internal Medicine, Ambulatory Division. He is dedicated to providing advanced, comprehensive, economical, kind and inclusive care for adults.
Jennifer Gilbert, DNP, APRN, FNP-C
Adult Primary Care Provider
Jennifer Gilbert is a certified family nurse practitioner with over eight years of clinical experience. She received her BSN from Fairfield University and began a nursing career in women's health, providing antenatal, labor and delivery, and post-partum care. Jennifer returned to school to obtain her Doctor of Nursing Practice degree from Quinnipiac University and transitioned to outpatient primary care. Jennifer began her APRN career in community health and has remained passionate about serving the population since. She incorporates her women's health experience in practice today, offering pelvic exams and pap smears at Family Centers, along with primary care to patients throughout the lifespan. SHe has also acquired training in HIV care, which she offers at the health center.
Lisa Henderson, APRN, CPNP-PC
Pediatric Primary Care Provider
Lisa Henderson is a certified pediatric nurse practitioner with more than 25 years of pediatric experience in primary, acute and emergency care at both academic and community health centers. She earned her BSN at Villanova University, MSN from New York University and is currently enrolled at Case Western Reserve University for her Doctor of Nursing Practice. Lisa has been a provider at Yale New Haven Hospital at Greenwich Hospital for 20 years and remains dedicated to providing comprehensive primary care in the community for children and their families.
Christine Higgins, APRN
Adult Primary Care Provider
Christine Higgins, APRN, is a dual-certified Family Nurse Practitioner and Certified Nurse Midwife. Christine holds a Doctor of Nursing Practice in Health Systems Leadership from Emory University. Her doctoral and clinical research focuses on mitigating environmental health disparities for pregnant individuals and their children. At Family Centers, Christine provides prenatal care, postpartum care, gynecologic care including contraception, and primary healthcare to adolescents and adults. She also provides sexual health screening and reproductive educational services through Greenwich Town Hall Department of Health every Tuesday afternoon.

Yekaterina Snelwar, APRN
Pediatric Primary Care Provider
Yekaterina Snelwar is a board-certified pediatric nurse practitioner with more than two decades of pediatric nursing experience spanning outpatient, inpatient and emergency settings. She earned her BS from the University of Connecticut and her MSN from Yale University. Yekaterina has been a trusted provider at Greenwich Hospital since 2000, serving in a variety of roles including pediatric hospitalist, emergency department clinician and currently as a pediatric APRN in the Pediatric Outpatient Clinic. She also brings experience from her work in the Juvenile Diabetes program at Weill Cornell Medicine. A native Russian speaker, Yekaterina is passionate about building strong relationships with families and supporting the long-term health and development of children in her care.
Susan Welch, APRN, CPNP-PC
Pediatric Primary Care Provider
Sue Welch has been a certified pediatric nurse practitioner for 29 years and has spent the past 22 years at the Greenwich Hospital Pediatric Clinic. She graduated from the University of Michigan with a BSN and from Yale University with an MSN. She is happy to continue providing pediatric care to the children of Greenwich and nearby communities.
John Midy, MBA
Director, Family Centers Health Care Medical/Dental
John Midy, MBA, joined Family Centers in 2018 and shares a passion in addressing the community’s health needs while fostering leaders in the team he manages. John leads Family Center’s Federally Qualified Health Center’s (FQHC) Medical and Dental operations. Since joining Family Centers, his dedication and leadership has expanded the agency’s services and increased access to care, helped the organization gain accreditation for Joint Commission and Patient Centered Medical Home, implemented innovative care team models, and increased agency brand recognition in the community. John graduated from Quinnipiac University and holds a Master’s in Business Administration with an additional focus in Health Care Management.
