Ramin Ahmadi, MD.

Dr. Ahmadi graduated from the Medical College of Wisconsin, studied Internal Medicine at Yale School of Medicine, and earned a Master of Public Health degree from Yale School of Public Health. He is a trusted medical and public health expert, and his insightful perspectives on healthcare have been featured in Healthline, Marketwatch, Cosmpolitan, and PBS Next Avenue. He has worked with numerous academic medical centers and community hospitals for more than two decades. As a clinician educator, he has been the recipient of many awards of excellence and has secured critical resources through highly competitive private funding receiving over $20 million dollars in federal grants to build accredited residency training programs in primary care.

Ishrat Rangwala, DMD
Dental Director
Dr. Rangwala received her Doctor of Dental Medicine (DMD) degree from University of Pennsylvania School of Dental Medicine. Prior to moving to United States, she received her Master in Dental Surgery (MDS), Prosthodontics from Gujarat University, India. She is a member of American Dental Association, Connecticut State Dental Association, National Network for Oral Health Access, and serves on the CT state’s CTDHP Dental Policy Advisory Committee. Dr. Rangwala was in private practice for over 5 years prior to joining FamilyCenters in 2016. Currently as Dental Director, she leads the dental program that serves to a diverse population. She excels in providing innovative, advanced, comprehensive care with strong emphasis on patient education and partnership. She is passionate about preventive dental care and integration of oral health with patient’s overall well-being. Each year, Dr. Rangwala completes over 50 hours of Continuing Education and has taken courses in prosthodontics, endodontics, oral surgery and infection control.

Joseph Hurtado, PA-C
Primary Care Provider
Joseph Hurtado is one of the primary care providers at the Family Centers Health Care. His educational background originated in Stony Brook University for his undergraduate degree. He then attended SUNY Downstate Health Sciences University to become a PA-C. Joseph primary operates out of the Family Centers Health Center located at 111 Wilbur Peck CT in Greenwich. Joseph is trilingual Spanish/English/Quechua and feels especially connected to his patients at Family Centers Health Care given his culture and background in medicine. One of his main goals at Family Centers Health Care is to help improve and provide healthcare access to lots of communities surrounding Stamford and Greenwich. Some of Josephs primary areas of expertise include nutrition, pain management, and primary medical care.

Cristal Sampson, APRN, MSN
Psychiatric-Mental Health Nurse Practitioner
Cristal graduated from Yale School of Nursing with a Master’s of Science in Nursing (MSN) in 2018 and has since worked in rural and urban outpatient community mental health centers. As an RN, Cristal has worked in inpatient psychiatry at Silver Hill Hospital in New Canaan, CT and St. Raphael’s in New Haven, CT.

Kathleen Sullivan, LCSW
Clinical Supervisor
Kathleen Sutherland is a seasoned mental health clinician who provides both supervision of and psychotherapy to individuals, families and couples of all ages. She is an integral member of the Family Centers clinical team crisis intervention services and community outreach activities. Kathleen has a wealth of experience working with wide range of populations in behavioral health inpatient, outpatient, and home based settings. She is certified in Dialectical Behavioral Therapy (DBT) and Functional Family Therapy (FFT) with a Master’s degree from Fordham University in Mental Health Counseling and a Bachelor’s degree in Psychology and Sociology from Quinnipiac University.

Health Center Director
John Midy, MBA, joined Family Centers in 2018, shares a passion in addressing the community’s health, economic, and social inequalities while fostering leaders in the team he manages. John currently leads Family Center’s Federally Qualified Health Center’s (FQHC) Medical, Dental, and COVID Testing/Vaccination operations. He also serves as Co-Chair of the Access Committee for the Greenwich Community Health & Improvement Partnership for lower Fairfield County region. Since joining Family Centers, his dedication and persistent leadership has expanded the agency’s services and increased access to care, helped the organization gain Accreditation for Joint Commission and Patient Centered Medical Home, implemented innovative care team models, and increased agency brand recognition in the community. John graduated from Quinnipiac University and holds a Master’s in Business Administration with an additional focus in Health Care Management.

Dennis Torres, JD, LCSW.
Chief Health Officer
Dennis Torres leads Family Centers’ medical, dental and mental health programs and services. Under his oversight, he has helped to create access to critical primary health, dental and health education services for thousands of Fairfield County residents. In 2016, Dennis led the opening of Family Centers Health Care – a Federally Qualified Health Center offering medical, dental and mental health services to public housing residents in Greenwich. He holds a Bachelor’s degree from Fredonia College, a J.D. from Pace University and a Master’s degree in Social Work from Columbia University.

Stephanie Haen, LCSW
Vice President of Behavioral Health
Stephanie Haen, LCSW oversees all clinical-based programs, including the Center for HOPE, Counseling & Mental Health Program, Trauma Response and The Den for Grieving Kids. In addition to providing psychotherapy to clients, facilitating crisis intervention services and supervising the agency’s clinical team, Stephanie implements oversees the maintenance of regulatory standards, serves as a liaison to community providers and engages in community awareness activities. She is also a level I certified AEDP therapist with a post graduate certificate from NYU in Clinical Supervision. Stephanie holds a Master’s degree from Columbia University and a Bachelor’s degree from Providence College.
