Here-to-Help

Here-to-Help (H2H) is a collective impact program designed to bridge the gap between students attending Stamford Public Schools (SPS) and essential resources and information available within their school and the local community. The program helps families succeed by providing access to resources to meet their immediate needs and connection to community services, including childcare, tutoring, nutrition support, technology, school communication and advocacy, and more.
We focus on supporting disengaged and under-engaged students attending SPS schools from Kindergarten through 12th grade by providing home visits as a first connection method. The program serves income-constrained families identified by school personnel as having chronic absenteeism.
Our bilingual Outreach Workers (OWs) complete an Individual Needs Assessment for each participating family to gain an understanding of any obstacles interfering with learning, and with family stability and well-being. The general screening details health, education, income, housing status, and social or civic connections.
H2H provides families with the resources needed to be successful community members, thereby reducing the burden on the SPS system. By linking families to these resources, academic performance, overall well-being, and involvement in the community are enhanced. Through strong partnerships and continuous evaluation, H2H provides comprehensive support to students and their families, helping them to thrive in and out of the classroom.
