Family Centers Health Care is a Federally Qualified Health Clinic (FQHC) located at 75 Holly Hill Lane and 111 Wilbur Peck Court (dental only) in Greenwich. We offer primary medical, dental and mental health services to children and adults living in Greenwich, Stamford and surrounding communities.
In 2025, Family Centers Health Care (FCHC) assumed management of the adult and pediatric primary care clinics formerly operated by Greenwich Hospital/Yale New Haven Health Systems, a part of an agreement between the two organizations intented to enhance primary care services for local adults and children and improve healthcare access for underserved populations. Patients of FCHC have access to Family Centers' comprehensive network of behavioral health, dental and human services to complement the robust network of medical specialist care from the Yale New Haven Health Systems.
FCHC serves all patients regardless of ability to pay. We accept Medicaid (Husky) and participate in most commercial insurance plans. Discounts for essential services are offered based on family size and income.

Medical Hours
Monday, Wednesday and Friday: 9 a.m. - 5 p.m.
Tuesday and Thursday: 9 a.m. - 8 p.m.
Mental Health Hours
Monday through Thursday: 9 a.m. - 9 p.m.
Friday: 9 a.m. - 5 p.m.
Our Services
Primary Medical Care
At Family Centers Health Care, we believe that primary medical care is essential in sustaining long-term health. Our Licensed Primary Care providers offer interventionary, preventative and lifestyle support to help patients live their best lives.
Primary & Preventative Care — Family and internal medicine, comprehensive well-child care, women's health and physical exams (school, athletics, employment).
Vaccinations & Screenings — Immunization and disesase screenings and management.
Acute & Specialty Care — Sick visits for adults and children and specialist referrals.
Dental Care
Proper dental care goes beyond maintaining a healthy smile—oral health impacts a patient's overall health. Family Centers Health Care's licensed dental team are equipped to handle all preventable, corrective and emergency oral health needs.
Preventative & Diagnostic Care — Cleanings (prophylaxis), comprehensive exams, full mouth X-rays and sealants.
Restorative Treatments — Crowns, root canals, extractions and complete/partial dentures.
Mental Health & Counseling
With mental health services embedded on-site, FCHC patients benefit from a "warm hand-off" to mental health clinicians, who help individuals, groups, couples and families resolve a wide range of personal, relationship and behavioral health issues.
Therapeutic Services — Individual, group and couples therapy for children, adults and families
Specialized & Evidence-Based Care — Evidence-based practices and trauma response.
Wellness & Prevention — Preventative mental health support and wellness initiatives.
For direct grievances, concerns and recommendations for change, please contact a program supervisor or director at 203-869-4848. If you are not satisfied with the resolution of your issue, then please send a letter detailing the grievance to the Family Centers Executive team at 40 Arch Street, Greenwich, CT 06830. A response will be sent in no more than 10 days from receipt of the letter.
