Family Centers Inc. is seeking competitive proposals for a General Contractor to perform interior improvements to create a /- 2,060 square foot School Based Health Center and girls’ locker room, at Turn of River Middle School, 117 Vine Road, Stamford, CT. The Owner will conduct two mandatory pre-bid conferences at Turn of River Middle School, 117 Vine Road, Stamford, CT on Tuesday August 13 or Friday August 16 at 10am. Bidders are only required to attend one session. Meet at the front entry of the school.
Each Bid must be accompanied by a Statement of Qualifications (SOQ) indicating the following:

Experience working on health-related projects

Experience working on projects with Federal funding.

Evidence of certification if a small business, minority or women owned enterprise.

Specialized experience, technical competence and qualifications relative to cost control, quality of work, compliance with safety procedures and capacity to accomplish this project in the required time frame.

Bonding capacity.
Four (4) hard copies and one digital copy of the SOQ and the Bid Proposal, addressed to Dennis Torres, Director, Healthcare Programs, Family Centers Inc., 60 Palmers Hill Road, Stamford, CT, 06902, shall be submitted in a sealed envelope plainly marked “Bid Proposal – School Based Health Center, Turn of River Middle School”. The SOQ and Proposal should be delivered to Family Centers Inc., 60 Palmers Hill Road, Stamford CT, no later than 9:30am, on September 4, 2024. The bids will be publicly opened and recorded at 10am that day.
Only Bids submitted on the General Bid Form supplied with the Bid Documents will be considered. All other Bids, including Bids received after 9:30am will be rejected. Bidders may be present at the opening of bids at 60 Palmers Hill Road, Stamford CT.
Should Family Centers determine that interviews are required, they will be scheduled via Zoom during the week of September 9-13..

Funding for this project is provided by the U. S. Department of Health and Human Services, Health Resources Services Administration. Attention of Bidders is directed to certain requirements of this contract which require payment of Federal Davis Bacon Act wages.
All project work must conform to the requirements of Turn of River Middle School, and the Stamford Public Schools.
Each Bid must be accompanied by a Bid Guarantee in the form of a certified check or Bid Bond in the amount of 5% of the total amount of the Bid. A performance Bond and a Labor and Material Payment Bond will be required in the amount of 100% of the total amount of Bid. No Bid can be withdrawn until sixty (60) days after the date of the opening of the Bids.
By submitting a Bid, each bidder agrees that it shall not bring any action seeking to enjoin an award of a contract for construction of the project or make any other claim whatsoever against the Family Centers Inc., the Architects, the Owner’s representative, or any other party engaged by any of the foregoing named entities.
The Owner reserves the right to reject any and all bids and to waive any error, omissions or other irregularities in the Bids received in the exercise of its sole discretion, acting in its sole interest.
