Dear Family Centers Health Care Patients,
We hope this letter finds you well. As we move toward finalizing our exciting new partnership with Greenwich Hospital/Yale New Haven Health, I am writing to share information on upcoming changes and improvements at our Health Centers.
Billing Statements: It has come to our attention that due to some system challenges; recent billing statements may have contained incorrect information. We sincerely apologize for any inconvenience and are working diligently to correct these issues. Updated statements will be sent next month. If you have any concerns or need immediate assistance, please do not hesitate to contact our billing team at 203-517-1085.
Registration Process Updates: We are currently updating our registration process to better integrate with our new electronic record system, Epic. These improvements aim to ensure a quicker turnaround time for scheduling appointments. We encourage all patients to utilize the Yale New Haven Health Epic MyChart platform, which provides real-time reminders of your appointments and easy access to your medical records. Enrollment Team Relocation: Please be aware that we have moved our Enrollment Team to the Billing Department located at 20 Bridge Street, Greenwich, CT. We will no longer be receiving documents at any of our clinic sites. We also want to reinforce the document requirements needed to access sliding fee scale discounts. A list of these requirements are enclosed. If you have any questions, please contact our Enrollment Team at 203-413-4469.
Demographic Information Update: As part of our efforts to maintain accurate records, we kindly ask that all patients visiting our clinic provide their legal name, date of birth, current demographic information, address, phone number, and payer information. We will verify insurance details for all insured patients to ensure we have the most recent information.
Participation with Insurance: We strive to participate with all health plans in the region. However, there may be times when a specific provider may not be in-network with your insurance. Please contact your insurance company to verify that we accept your individual plan. Additionally, follow up with your insurance plan to confirm your coverage and benefits for the services you plan to receive.
Transition to New Electronic Record System: We appreciate your patience as we transition to our new electronic record system. Our team is diligently working to retrain staff and identify the best approaches to support our patients and provide the best possible customer service experience.
