Please complete this form for each child currently enrolled in a Family Centers Early Childhood Education program — even if your child will not be returning next year.

To secure your child’s space for the 2027–2028 school year, please submit the form by December 1, 2026. Enrollment is first come, first served, and space cannot be guaranteed after the deadline.
