Health Care

Family Centers Health Care is a community and school-based program that provides comprehensive medical, dental and mental health care to adults and children living in Fairfield and Westchester counties.

Our Approach

We are committed to high-quality, patient-centered care, using the latest, evidence-based medical treatments.  We treat each patient with respect and dignity and focus on helping people feel better.  We regularly review our practices to ensure our care stays high-quality and are guided by input from our Patient Advisory Board.

PERSONALIZED, HOLISTIC CARE

Our focus is on prevention, overall wellness, and ongoing, personalized support.  We use a team-based model of care where a lead provider works in close coordination with the patient and a team of nurses, social workers, and other specialists to coordinate all aspects of care. 

HEALTH CARE FOR ALL

We believe health care is a fundamental human right.  We participate in most major insurance plans, including Medicaid (Husky), and serve all patients regardless of insurance coverage or ability to pay.  Fees are assessed on a sliding scale based on family size and income.
