Family Centers Health Care is licensed by the State of Connecticut and receives funding from the U.S. Department of Health and Human Services (HHS). It holds Federal Public Health Service (PHS) deemed status, which extends to certain health or health-related claims, including medical malpractice claims, for the organization and its covered individuals. HIV Services

Since 1989, Family Centers CARES (formerly Stamford CARES) has been a leading provider of HIV prevention and treatment services across lower Fairfield County. Our multi-service program addresses barriers and disparities so people living with or at risk of contracting HIV have access to life-saving prevention tools, medical care, and long-term treatment.

Our Approach

Family Centers CARES serves individuals and families living with HIV with incomes up to 400% of the federal poverty level. Each client works with a medical case manager as their primary point of contact to schedule appointments, navigate services, and address barriers to care.

Intensive medical case management for newly diagnosed

Outpatient medical care from providers skilled in HIV treatment
Access to housing assistance, transportation services, direct emergency financial assistance, and help with public-benefits applications and enrollments
